Referring one of your patients to a specialist should have as little impact on your workflow as possible. With this in mind, we want to make referral easy for you and your staff. To refer a case to Hope Veterinary Specialists, simply fill out the referral form below, or download the form as a PDF fax and fax it to us at 610-296-2444. If you wish to speak with a Specialist regarding the case prior to referring it to us, please just give us a call at 610-296-2099 and we would be happy to assist you!
For any patients being referred to our emergency service, please call 610-296-2099 to speak with one of our ER doctors or criticalists regarding the case. You may also utilize the online/fax submission form, however, we kindly ask for a courtesy call to inform a Hope VS client specialist the form is being emailed/faxed so we can better serve your client.

What to expect from us
Communication: We understand that patients receive the best care, and clients are best served, when the referring veterinarian and veterinary specialist work together in a collaborative manner. This is accomplished through efficient, timely, and consistent communication. Hope Veterinary Specialists is committed to excellence in communication with the referring community. In addition to receiving daily SOAP reports on hospitalized patients, we will send you a final referral report in a format that is the most convenient for you (e-mail, fax). In house blood work results are summarized daily in the SOAP and sent in full detail upon patient discharge. Laboratory tests sent to an outside service, such as Antech, will be faxed to your hospital upon completion of test. If you wish to speak with us at the conclusion of the referral, we would be happy to accommodate you!
Compassion: When your clients are referred to Hope, their beloved pets will be cared for by a team of veterinary specialists and dedicated nursing staff. Your clients will also have the benefit of sharing their concerns, discussing treatment options, and end-of-life issues with Jennifer Durn, MSW, LSW, who serves as our Support Services Coordinator and is available in person and by phone to lend support.
